About the Agency:

MedAssist empowers patients facing medical challenges to thrive by addressing their needs with comprehensive access to services through care management. Our Agency is expanding and we are looking to add some great people to our team!

Position Overview:

You will be supervising care for patients who are enrolled in MedAssist services including chronic care management, behavioral health, and advanced care planning. We want to assist our patients with enhancing their health & overall well-being by building lasting connections with them - this starts with the tone set forth during the initial visit. This position reports to the Chief Operating Officer.

General Responsibilities (including, but not limited to):

• Ensure all medical services align with the Agency’s mission & vision.

• Provide medical feedback relative to patients’ continuity of care.

• Participate in cross-departmental meetings, e.g., case conferences, and patients of concern.

• Perform all other related duties as assigned.

• Conduct one 30-minute telehealth visit per patient and document the visit. After completion of the initial visit, the Care Management team will be checking in weekly with the patients to see if and how our team can help connect them with resources.
• MD or DO will be compensated at $50 per completed telehealth visit.
• Visits must be completed between the hours of 8 am-6 pm Monday-Friday, possibly open in the future to weekend work also.
• MD must have at least 20 hours of availability per week.

Requirements and Qualifications:

• Current license and certification to practice medicine (MD/OD); permanent licensure in the State of New Jersey.

• Well-versed in EHR.

• Positive Attitude. Reliable & dependable.
