Constellation Health Services is currently seeking a Registered Nurse - RN Supervisor to join our team. The ideal candidate will display compassion, education, proficiency, and experience while assisting the patient. This critical role assists in creating and managing nursing care plans and will participate in the evaluation of the current and future medical needs of the patient.

Responsibilities:

• Provides for maximum utilization of staff to ensure optimal delivery of resident care.
• Assures that all units are staffed appropriately according to established guidelines and covers staffing using the available resources. Replace cancellation call-outs from staff.
  
• Admission, transfer and discharge for complete and accurate identification, documentation and appropriate plan of care.
• Responsible to start the investigation process when (incident/accident, altercation resident to resident, incident of unknown origin, skin tear bruises etc.) Occur and to follow guideline for proper documentation.
• Supervises and instructs all personnel on all new procedures, policies, treatments, and equipment.
• Provide exceptional patient care – Maintain nursing standards while supporting patients with all applicable medical needs. Create and provide patient care plans as needed or requested. Communicate with the patient, their family and members of the medical team to achieve the desired goals of the patient care plan.
• Conduct administrative duties – Attend medical conferences and courses for ongoing care and education. Assess patient care plans and identify areas of improvement to ensure quality of care.

Qualifications:

• License and current registration to practice in the State of New York as a Registered Professional Nurse.
• Associates Degree in Nursing, Bachelor’s Degree preferred.
• COVID Vaccinated
• A minimum of six months of recent medical experience.
• Strong interpersonal and written communication skills.
• Ability to be self-directed and work independently.
• Thorough knowledge of current computer software (ie. Word, Excel, smart sheets)
• Able to work in close contact with Department Heads, Management staff, and the community.
• The ability to work effectively in high-activity situations and under pressure.
• Innovative and motivated to continuously improve internal systems and processes.
• Have a valid driver’s license, a reliable vehicle, and willing to commute to provide the best in-home patient care.
• Willing to work a rotating on-call (via telephone only) to triage any incoming emergencies.
• Some weekend days are required, and summer and winter holiday commitments as needed
